Native Name, Logo, or Mascot
Kindergarten through 12th Grade

Step 1 – Hearing Schedule Page
Go to ILGA.gov hearing schedule page and click the “Create Witness Slip” button.
Step 2 – Look for Bill #: HB5617
On the next page, look for Bill #: HB5617 and click the pencil icon on the right side of the row.
Step 3 – Identification
- Name, Address, City, State, Zip: Complete these lines (no dots or abbreviations)
- Firm/Business: “none” or “self” or name of organization if you are empowered to act on behalf of the organization
- Title: “self”
- Email and phone: Complete these lines
Step 4 – Representation
- Representation: “self” or name of organization if you are empowered to act on behalf of the organization
Step 6 – Testimony
- Testimony: Select “RECORD OF APPEARANCE” – Record of Appearance Only
Step 7 – ILGA Terms & Submit
- ILGA Terms of Agreement: Check box to agree and the click Create (Slip)
