On Friday evening, join us at the Community Recognition Event as we honor and celebrate the incredible individuals who dedicate their time and talents to uplifting the Chicago American Indian community. This special event will recognize the outstanding contributions of our volunteers and bring together community members for an evening of appreciation and connection.

On Saturday, join us for our 10th annual Urban Native Education Conference where we will facilitate the exchange of strategies, tools, and research for Native American education in urban communities. Interactive workshops will be offered on various educational issues, broadly construed, tackling the annual theme of Beyond Borders and Binaries.

Will you require childcare? We offer free childcare services for youth ages 4 to 13 years. They are welcome to join you for the conference program, however, if you would like them to participate in our childcare services, please complete the registration below with their name(s), age(s), emergency contact info and signed waiver.

CHILDCARE WAIVER, RELEASE OF LIABILITY, AND CODE OF CONDUCT

I am the parent or legal guardian of the child(ren) listed below and hereby acknowledge and agree to the following terms and conditions related to participation in childcare services provided during the 10th Annual Urban Native Education Conference, hosted by the Chicago American Indian Community Collaborative (CAICC).

Childcare Participation & Assumption of Risk

  1. I understand that CAICC is offering free childcare services for youth ages 4 to 13 years during the conference and that participation is voluntary.
  2. I acknowledge that while CAICC staff and volunteers will provide reasonable supervision, participation in group childcare activities involves inherent risks, including but not limited to minor injuries or accidents.
  3. I voluntarily assume full responsibility for any risks associated with my child(ren)’s participation in the childcare program.

Medical Authorization & Emergency Care

  1. I certify that my child(ren) are physically able to participate in age-appropriate childcare activities.
  2. In the event of an emergency, I authorize CAICC staff and/or volunteers to seek necessary medical care for my child(ren). I understand that reasonable efforts will be made to contact me or the emergency contact listed.
  3. I understand that I am financially responsible for any medical treatment provided.

Release of Liability

  1. I hereby release, waive, and discharge CAICC, its board members, staff, volunteers, partners, and representatives from any and all claims, demands, liabilities, or causes of action arising out of or related to my child(ren)’s participation in childcare services, except in cases of gross negligence or willful misconduct.

Parent/Guardian Responsibilities

  1. I agree to:
  • Provide accurate emergency contact information.
  • Pick up my child(ren) promptly at the designated times.
  • Inform CAICC staff of any allergies, medical needs, or behavioral considerations.
  • I understand that children may not be left unattended outside of the designated childcare program.
  • CODE OF CONDUCT (Applies to all attendees, volunteers, staff, and participants)

    Violations of this Code of Conduct may result in removal from the childcare program or conference space.

    Respect:
    All participants are expected to treat one another with respect. Discrimination, harassment, bullying, lateral violence, or intimidation of any kind will not be tolerated.

    Safety:
    The safety of our community is a shared responsibility. All participants must follow posted rules and staff instructions.

    Privacy:
    CAICC respects the privacy of all attendees and expects the same in return.

    Weapons:
    In accordance with Illinois State Law, weapons of any kind are prohibited at all CAICC events.

    Prohibited Conduct Includes (but is not limited to):

    • Physical or verbal threats
    • Harassment, bullying, or coercion
    • Sexually explicit language or obscene gestures
    • Racial, religious, gendered, or ethnic slurs
    • Leaving children unattended
    • Defacing or damaging property
    • Fighting, public disturbances, littering, or loitering

    Photography and video recording may occur during the event. Please notify CAICC staff if you prefer that your or your child’s image not be used in promotional materials.
